Job Description

Mindful Care is seeking a Clinical Intake Specialist (LMSW) , with deep clinical knowledge and background, to provide initial mental health interviews and evaluations for patients seeking treatment at our office in Manhattan or West Hempstead. Knowledge in understanding the implications of the Biological, Psychological, and social factors of our patients' lives and experiences is expected to make recommendations for the services we provide, as they embark on their mental health and/or recovery journey with us. Our LMSWs will fully assess and collaborate with patients and other members of the clinical team to ensure that patients are educated and have a strong treatment plan to follow at Mindful Care.

Our Clinical Intake Specialists:

  • Perform Initial Evaluations (90791s) for all patients seeking care at Mindful.
  • Become SMEs on the variety of services at Mindful and help formulate Treatment Plans based on individual needs of the patients.
  • Attend clinical meetings.
  • Provide coverage for the Therapy department when needed.
  • Participate in department training and collaborate with other treatment providers.
  • Following strict ethical and clinical guidelines.
  • Exhibit a patient-first / patient-focused mentality.


Your Qualifications:

  • A master's degree and valid licensure to practice in New York (LMSW).
  • Strong patient assessment skills, including mental status exam and use of screening tools.
  • Experience using PHQ9 and GAD7 screening tools is required. Must be very familiar and comfortable using these tools.
  • Knowledge of DSM-V.
  • Strong communication, patient/customer care, and leadership abilities.
  • Ability to organize and time-manage.
  • Team-player.
  • Desire to work with diverse populations and ages 12-lifespan.
  • LGBTQ ally.
  • Bilingual, a plus.

  • Type: Regular Full-time, Exempt
  • Schedule: Hybrid - Monday-Thursday, 9am-7pm, in-person 2 days
  • Location: New York, NY or West Hempstead, NY
